How to unlock the They'll Tell Stories About This achievement in Outward - Definitive Guide
To get this mammoth of an achievement you are required to finish one of the main storyline quests, without ever be beaten down to 0% Health or pass out by hunger thirst or weather conditions. (since it came up as a question in the comments I like to elaborate, that you only can get this achievement if you never "die" at any point untill the achievement pops, so watch out at the start of the game too!)
It is recommended that you do this as a second playthrough so you have a better understanding of the games mechanics and where everything is in the game.
I would recommend the Blue Chamber Collective for the fastest and easiest completion of the three main questlines.
For this simply follow the objective that is given to you from Rissa Aberdeen in the first town.
A full Walktrough is beyond my capabilities but Id like to give some tips for a good start and a solid base for the rest of the game:
I went with a Meele focused build on my runs which I would also recommend, since the magic system is really complex in this game and If you haven't allready played as a mage before, you will just make it harder for yourself.
For an easy and fast start I would recommend, taking everything that you find in your house at the start (don't forget the absolutely vital flint and steel!) and then going through the dark cave in the Harbour area of the first town with a bandade or a healing potion in your inventory. (you should find a bandage in your house)
In the cave are two troglodytes that you have to kill, but they shouldn't be a challenge.
If you venture through this cave you'll eventually find yourself on the edge of a Plattform that you can only jump down and don't get up again but don't worry, jump down the Edge and out of the Exit of the cave.
You'll find yourself on a beach, here you just have to go along the shore (avoid the shrimps, they hit hard!) and you'll eventually meet Michel Aberdeen, injured and unable to walk.
Talk to him and give him the potion or the bandade to heal him and he'll will give you a Tribal Favour in return. this favour can be given to Rissa Aberdeen to pay your debt without having to pay the 150 Silver. With this Money you can buy viable equipment for yourself or you could go to the town's skill teachers on the top of the wooden structure and learn from the skill teacher Eto Akiyuki, he can teach you a skill that increases your Health and Stamina 15 Points wich is a nice Bonus to your survivability.
When you make your way back to Cierzo you can make a little detour to the Immaculate's camp in Chersonese, to get a really good starting Backpack. The Adventurers Backpack. For this you have to ask, if the Immaculate could help you and then say that you Need storage. (Theres a chest at a tent in the cave too, don't forget to loot it!)

There is also a "hidden" sidequest in the first town. If you go to the gate of the town and then head Right and up a little wooden staircase you'll find Helen Turnbull wich asks you to bring her a Mushroom Shield from an Trog-Cave with the Name: Blister Burrow (This is also marked on the linked map) if you bring her the Shield she'll give you a Pretty good amount of silver plus you can sell all the loot of the cave. (she also takes the giants heart that you can find in Blister Burrow for more money than any of the traders in town)
I would also recommend, that you try to craft a "Fang" weapon as quickly as possible because they cause bleeding Damage wich can be really helpfull with the early game since you can Slash an enemy and then just cirlce around them until they die. (although it has been nerved in a recent patch)
A neat thing to mention is, you don't need a recipe to craft an item In the games crafting system, if you can remeber what you need and have every required item you can craft it without the recipe.

If you completed a questline before starting this run you can give yourself a serious boost in attack, scroll down to the asterisc (*) to learn more!
Another godsend are the Plant Tents, always keep one or two on you.
These tents are one use items only, but provide you with a tent, that practically works as good as your bed in the first town, so no hunger or thirst if you rest in it.
They also stay forever where you've planted them.
You can actually "Heal" your hunger and thirst if you Sleep in one of theese badboys! But they are not immune to ambushes so keep an eye out for that!
Try to find spots on the map with blue butterflies, these are safe-zones where you don’t have to put hours in guarding to protect yourself.
But remeber! some quests are timed so check the discriptions before you Sleep day after day!
Always keep the weather in mind! In some places it’s hot in others cold you can survive quite a long time in hot weather as it only affects your stamina consumption as long as you stay in the yellow icon, if your icon turns red the heat also affects your physical damage resistance and gives it a -60% multiplier, something to keep in mind if you fighting in hot places (especially in the desert) but cold temperatures are far more common and also damage your health when you reach the red icon.
Luckily cold is pretty easy to combat; every source of fire heats you up a bit.
A torch or a campfire everything helps.
And your temperature depletes slowly, so you can kind of store a bit of warm "energy" venture for a bit and the warm up again on a campfire (every tree in the game can be harvested for the needed materials for a campfire) there are Season changes so be prepared for this also. (they most often bring cold weather)
You can buy equipment that protects you from cold pretty well in Cierzo but you can also use food, potions and various torches to cool down or heat yourself up.
Always be prepared before you engage in a battle. theres is plenty of Food, that gives you bonuses on Health regen, elemental resistance, physical resistance and so forth, don't be afraid to use them!
I for once have some kind of meat on me at every time, because they give Health regen. Combine that with bandages and you heal a pretty good amount of health in a relative short time.
To heal the penalty on your health and stamina that you will eventually get, its also a good idea to keep some tea on you. they can also be bought at Cierzo
But remember the plant tents! I can't stress enough just how usefull these things are, I usually put one in every town, so If im struggling with my condition I just run back in town and heal myself without the fear to get ambushed or starve/die of thirst.
Always asses a Situation before engaging, If I learned something in my attemps then that would be, if something looks tough to fight it almost all of the time IS tough to fight so don't take on the biggest enemys just from the start.
This game doesn't have a lvl System, avoid fighting if its not necessary and only engage enemies if you are confident that you're gonna defeat them.
And don't hesitate to run away if you know you will lose the fight.
All enemies (except the Bosses) are gonna loose interest in you, if you run away Long enough, use this to your Advantage!
Another tip from Rippi Redd in the comments:
If you have a friend, that already has the achievement and end-game gear you could go in coop and transfer the equimpent over to the new character.
Since there's no leveling in Outward you can equip the gear right away and have a huge boost on survivability.

Additional to that, the game has a system called Legacy Chests, where previous characters can put stuff in special chests in the game, so new characters can get the item put in the chest (these Items even "lvl-up" in these chests and get new appearences and stats!) the easiest to reach for a new character is in Cierzo's town hall, but you have to get the storage key from Rissa first wich requires you to either join the Blue Chamber faction or completing the Vendavel quest. (wich is a bit of a head-scratcher in it self, but I'll be getting to that a bit further down in the solution)
I would recommend to get the key for the chest in your first playtrough and putting a Cleaver Halberd (wich you can get from the Bandit Camp leader in Cierzo's Montcalm Fort) because the Legacy variant has extreme bleeding as an effect, wich is really good and then getting the key with your new playthrough by joining the Blue Chamber.
From here on it’s up to a bit of luck and huge amount of patience, I wish you good luck on going for this beast of an achievement!
One last thing! BEWARE: there is an event at some Point in the game, where you are told, that bandits want to raid cierzo, where you have to fight off the Bandits in their castle. If you choose the wrong dialog Option when you're there, you'll be thrown in jail without the possibility to defend yourself. I cannot say if this Counts as an "defeat" but to make sure I would watch out for this "Vendavel" quest.

This Information is old since the achievement has been patched
I've completed the game two times now without ever being "defeated" and it didn't Pop.
Apparently it is bugged on PC as well:

"Sometimes the achievement 'They will tell stories about this' was not unlocked properly. It is due to completing a quest line that was previously completed by another character. This has been fixed and will be included in Experimental Branch really soon. "
This was posted in the thread, that I've linked on the 16th of July by the developers. But this comment was made on the PC Version of the game, so I can't say for sure, that this is also the case for the Xbox Version.
Edit: (15.11.2019) a couple of days ago a new patch has been dispatched for outward, adding new weapons and Armor to the game. But with it came a fix for this achievement and you can now earn it even retroactively. I’ve loaded a save game, that never died in the campaign and got it right away when the loading screen went through. So if you still have a save game with a character, that has done the main quest without dying just load it up and you should get it!
Missable
Complete the game from Character Creation up until finishing the final Faction Quest without ever fainting. As soon as the quest log updates to Well-Deserved Rest, the trophy pops if you did not faint.
You faint by having your HP reduced to zero, by environmental factors (overheating or freezing outside) or by other means. You will recognise fainting by the accompanying Defeat Scenario, a small picture and explanation of what happened after you fainted. Some times you get captured by bandits, other times helpful town folk carry you back to safety.
During Looking to the Future, you might opt to roam the world before choosing a Faction. Collect gear and skills, flesh out your build before taking on the Faction Quests.
Reports on the web say you do not need to include your Faction’s Parallel Quest in this run. During my playthrough, I unlocked the trophy during a run where I did complete the Parallel Quest so cannot confirm or deny this as of yet. I will update when I know more.
In the Road Map you are suggested to try this when siding with the Holy Mission. In that final quest, you have the option to either join the final fight against several difficult monsters, or sacrifice yourself. By choosing the latter, you get to fight some very weak monsters, avoiding the harder fight outside. Sacrificing yourself does curse you, decreasing your maximum health as the in-game days pass (only in your game; when joining someone else in co-op, your curse will not be there). If you plan to continue with your character, you might not want to attract that curse.
Note that playing in splitscreen or online co-op makes this trophy easier since you will only get a Defeat Scenario when both players faint. If only one player faints, no Defeat Scenario will occur and you can still acquire this trophy.
