Edit3: Reaper has been changed twice since I posted this. He now reloads when he Wraiths and when he Death Blossoms. Comments confirm Death Blossom kills still count, however.
Your best bet when going for this achievement is to try to make your first kill on your first shot after reloading. As far as we can tell so far, a solo kill is where no one else gets an assist. You may not necessarily do 100% damage, but it's been long enough since the person got hurt for no one to get an assist. People have inconsistently been getting credit for using Reaper's Ult as well.
Other than that, your best bet is to look for the squishy characters - Widow, Mercy, a D.Va not in her Mech, Hanzo, Zenyatta, etc. Essentially anyone with a small frame. Tracer is pretty weak but her movement makes it hard to get a good shot in. You actually want to AVOID helping teammates, as you're just wasting shots you need for your solo kills.
At least for me, I was in a fight where we both had to reload at the same time, so I was able to get a kill on my first shot. Sneak behind the the enemy, or wait somewhere you don't think theyll see you if you're on defense. Get them while they're distracted. Avoid reloading. Only reload if you wasted too many shots on your previous kills.

I got three full kills in one load by my own, but no pop. I guess there is a time limit because I took some time to find the last two.

Yeah this definitely seems to be one of the least clear achievements on unlocking correctly.

Reaper's Hellfire Shotguns are his main weapons with 8 ammo, dealing UP TO 140 damage per shot with a headshot.
We will need to kill 3 enemies without reloading and having the kills register as "solo kills". Obviously, the perfect way to get this involves 3 enemies all being killed at 100% although these scenarios are rare. A solo kill is a kill that you did the most damage to (over 70), got the final blow on (name in kill feed), and did not get buffed by a teammate in any way (no teammates got assists).
Keep in mind that this achievements seems to be very inconsistent with many people reporting that they got 2 shotgun kills and then a death blossom kill (without reloading) to unlock it.
I recommend going for this while attacking and trying to kill enemies that newly spawned, or enemies sitting at the back that have low total healths. I personally got it by getting 3 "100" kills on Pharah, Junkrat & Hanzo inside the enemy base.
One good strategy to have is trying to get your first kill on a new clip, meaning that you will have 7 ammo to get the remaining 2 kills. Damage and enemy to low health, reload and finish them off to start the chain.

1.) Que up for quick play with your friend and let one of you go to your icons in the upper right corner of the screen.
2.) Choose whoever may want to volunteer to be on the opposing team by having them hover over “leave group”, and do NOT yet leave until later.
3.) When you find a game, let the person going on the other team leave from the group immediately. Follow the next steps if they are on the other team. If not, repeat the previous steps. This may take multiple attempts.
4.) One of you should choose Reaper, and someone should choose Sombra (quickest way to do this achievement).
5.) Find a private location on the map, where nobody on the teams except for yourselves will see you.
6.) Let Reaper get a very close up headshot with one click on the controller on Sombra. Then repeat two times, without reloading. It should then pop up. Good luck!

I got this accidentally during a comp game.
Was playing Flashpoint, we got a team wipe.
I ran towards the enemy teams spawn and Death Blossomed in the middle of the team, killing all 5 of them at once.
They took zero damage from any other player, and I didn't shoot before using my ultimate so I assume it counted as the three solo kills without reloading.
Regardless, it's a strat to try, and I hope it works like that for you, because it's one of my proudest Pixel sprays along with Baptiste's.

You need to do just single kills, carried out by you from start to finish, without the help of your allies.

Incorporeality now restores the entire clip, which automatically resets the trophy counter.
