TRIALS
In order to even enter the Eternal Labyrinth (code-named Uber Lab by players), one must unlock each of the Trials which are found post-campaign in Maps.
Any standard map or Zana mission map has a random chance to spawn one of the 6 Uber Lab Trials. They show up on the mini-map as a large green portal with a Skull next to it. They are advanced trap rooms like the ones you did to complete the previous Labyrinths. They generally have 3 sections each. These are:
Trial of Piercing Truth -- Floor Spike Traps
Trial of Burning Rage -- Lava Floor
Trial of Lingering Pain -- Floating Death Roombas
Trial of Crippling Grief -- Small Floor-Based Saw Blades
Trial of Stinging Doubt -- Poison Dart Traps with a random assortment of other Traps
Trial of Swirling Fear -- Spiked Spinning Totem Poles
Dying inside of one of the Trials will not use one of your portals for the map. If you are playing Softcore, you can attempt it as many times as you want and resurrect at the start of the Trial if you die.
Upon completion of any of the Trials by touching the Stone Tablet at the end, an Offering to the Goddess will drop (only one will drop even in group play, and it will be filter-locked to the owner of the map if he or she is present in the Trial). This is a consumable key to enter Uber/Eternal Lab from the Aspirant's Plaza. You may need multiple keys to complete the Uber Lab. If you die or disconnect while inside, it will start you from the beginning when you use a new key. For group play of the Lab itself, only one key is needed for the whole party, but it will make Izaro much harder to kill.
It is possible to get the same Trial multiple times even if you have already completed it. Doing one you've already completed will net you another Offering to the Goddess, but won't affect progression toward unlocking Uber Lab. So, you either need to get lucky or boost it with friends doing maps separately from you in order to unlock all six. You can enter a friend's map and do the Trial even if they did it prior to you entering. You will still have to run through it though as a portal cannot be dropped inside the Trial.
You can view your progression on the Stone Tablet at the end of the Trial or on the one near the Trial's portal inside your map.
LABYRINTH
Once you have all six Uber Lab Trials done, you can enter the Uber/Eternal Labyrinth through the Aspirant's Plaza. You need to have the Offering to the Goddess in your inventory as there is no stash in the Aspirant's Plaza. There you can see where Silver Keys/Chests, Puzzles and Gauntlets for Labyrinth Troves, and (more importantly for this completion) Darkshrines are. Darkshrines provide permanent bonuses throughout the Lab. You can read which random effect you've been given once you hit a Darkshrine by applying the full mini-map and looking in the top right corner of your screen. All Darkshrine bonuses are positive. I would suggest hitting every Darkshrine you can find in the hopes of getting either "Labyrinth Traps are Disabled in the Aspirant's Trial" or at the very least "Players Take 50% Reduced Damage and Damage Over Time from Labyrinth Traps." Either is helpful in the final Izaro room, though the former is preferable. Darkshrines are noted in particular rooms on the poelab.com layout, but Labyrinth rooms can be expansive. You should carefully look at your mini-map while exploring a room you know has a Darkshrine for a hidden alcove off the wall. There will be a switch which opens the wall behind which is the Darkshrine and a monster pack.
Not to overlook the difficulty, the monster level is 75. That doesn't mean that you should necessarily be able to complete it once your character is 75. Monster level 75 is the same as a T8 Map. However, Izaro is probably closer in difficulty to a T10-12 Boss. If you can do T10-12 comfortably and kill the boss (without dying, mind you), then you should be able to blow through an Uber Lab without too much difficulty. This might not happen until your character is over level 80 depending on your build. It is just a longer version of the Labs you've done previously. You can expect to spend anywhere from 15-45 minutes in Uber Lab depending on your navigation skills and build, of course. Though there are Uber Lab Farmers who make builds just for it that can run it in 5 minutes or less.
Once you've killed Izaro in the third Aspirant's Trial, enter the treasure room which will have opened. The Altar of Ascendancy near the top of this room will need to be hit to access the last two Ascendancy points and unlock the achievement.

Is there any rhythm or reasoning to what trails spawn where? I've only been able to find 3 so far after 400+ maps lvl 1 to 7

The Eternal lab is unlocked from completing the 6 different trials found only in maps. Other than that, the Eternal lab functions the same way as Merciless lab, just harder.
