How to unlock the Know When to Fold'em achievement in Psychonauts 2 - Definitive Guide
At the end of the casino level you will encounter the luctopus.
She will fire explosive bulbs at you periodically, use telekenesis to pick them up and hold them until a hole is visible on either the right or left tentacle (which ever is the target).
Hitting this will allow mental connection nodes to appear which will take you to a platform to the side of her (same side as the hole). Shoot the top of the head with a blast which exposes a trail. Follow until you are above the head and drop, smashing the heart.
Repeat 3x. Achievement unlocks after the cinematic.
Only thing to watch for is the card shuffle which shows the lines on the floor the cards will take, and an electric shock where you need to stand on the poker chips.

This didn't unlock for me after I beat that boss... I unlocked the level achievement after I completed the level but... why didn't this unlock?
So it turns out that QUICK RESUME glitch it true; delete local saves/reinstall and MAKE SURE that if you think you're going to load something else MAKE SURE to PRESS START and QUIT GAME. Unlocked for me after I did the fight again.

Upon re-entering Hollis's mind, you'll be inside the world known as Hollis's Hot Streak. The neon lights and gambling themes are the result of Raz's meddling with Mental Connection in her mind. At the end, you'll fight an embodied personification of Lady Luck, combined with Hollis's new personality, and she has kidnapped the other Interns to hold against you.
There are three phases to the fight, but each phase plays out similarly. Use PSI Blast to expose a sucker on the tentacle holding your classmates, then wait until Luctopus uses the Bomb card. She'll toss out a few that will be marking where you stood when she tossed them; grab one with TK (Telekinesis) and toss it into the exposed sucker socket. This will release the Interns and allow you to use Mental Connection to reach into her mind/heart. From here, simply melee the heart until she forces you out, rinse, and repeat. Some challenge comes in being able to hold on to one of the Bomb plugs, as if you grab one of the earlier ones sent out, the next might collide with what you're carrying and explode it. Later phase additions include her briefly covering the tentacle with a hand of cards or interchanging which side the carded Interns are in.
