Oh and anytime there are ammo crates that respawn (usually during boss battles), I recommend just rolling around the room repeatedly while you fill up on ammo.
Also if you save your red gems, do your health upgrades during boss battles when your health is low, it automatically fills up your health meter.

Your gems should go into upgrading the damage/capacity for the machine gun and the reload/damage of the shotgun. Every 3 red gems you pick up, put a point in health (make sure you only upgrade health when you're low on it so you get a free heal)
Don't worry about upgrading anything else. The boner and torch are useless. I'd put a point in the light shot's rate of fire and enemy stun time, but no more than that.
There's a reason you shouldn't be putting all of your points into health (if you're not going for all of the red gems, which you probably aren't on hard). Instead of buying a ton of red gems with your money, buy the absinthe packs at the shop. You can get 6 absinthe's for the price of one red gem, and they heal for 200hp (basically a whole maxed out life bar). You will save a ton of money this way. Tequila's come 3 in a pack for the same price and heal for 70hp, so those of you with smaller life bars will benefit more from those.
Be aware of this: if you collect a red gem and hit a save point, and you're able to go back to the location of the red gem, hit start and quit. when you load your game back up, the red gem will have respawned and you can cheat the system this way sometimes.
If you still find it too hard and want a cheap way to grind, in chapters 3 and 4, there are enemies that teleport around. If you can hit them with a light shot in the legs, sometimes they'll fall down. when they're on the ground, you can hit x to stomp them until you have 999 white gems.
As far as boss strategies go, there's no new patterns on hard. Enemies do more damage and take a little bit more. Overall relatively easy once you know boss patterns.
Previous guides help through the vast majority of the game, I'm only going to focus on the last 2 Chapters of Act 5. I will say that upgrading weapon damage and capacity are key, I didn't increase reload speed on any weapons, they all reload just fine by default. And once damage/capacity were fully upgraded I used all white crystals to purchase health bottles instead of red gems. There's also really no need to spend white crystals on ammo, there's plenty throughout the game. Trust me, this difficulty is not the one to use when going for the achievement where you acquire all red gems in the game.
The boss fight against Fleming is so BUNK, I tried every solution mentioned here and in the comments, and none worked. I was excited about other suggestions to allow the green dome shield to be destroyed by Fleming, but that didn't work for me because if I let Fleming destroy the green shield himself, he never reached the damaged state where he stands slumped and still. And no matter how many times I hit him with my hot boner, sticky payload, shotgun and/or macine gun (all upgraded, mind you), I couldn't get him to show the gold cracks in his armor. What eventually worked was the following.
I made sure my shotty (with max upgraded damage and capacity) was fully loaded. I launched a cannonball into the green dome and immediately charged forward to it without taking the time to reload. This is important because there's not much time between shield destruction and Fleming resuming his attacks. When the green dome exploded I was not harmed as it took all the damage, and I was positioned only a few feet from Fleming. While advancing I held LT/RT to load up 4 shells into the shotgun for max damage, and the 4th was in the chamber just as Fleming got into the stunned state. I hit him with all 4 shells, at which time the gold cracks appeared. DO NOT hold RT long enough to allow the cannonball to fire, make sure you release all 4 shells at him then immediately switch to the Hot Boner. He shoud show gold cracks at this time so stick him with a sticky payload and hope you have time to detonate it. I had to do this about 15 times before I could perform all the steps quickly enough, and after a long, hard fought battle, he finally went down. This was VERY frustrating to say the least, just an aweful way to (nearly) finish a game. Part of the problem was sometimes, even after hitting him with all 4 shotty shells, he wouldn't show gold cracks. The other problem was that the controls are also glitchy and many times, when the gold cracks did appear, I would hit left on the D-pad and it often WOULDN'T SWITCH to the Hot Boner at all. UGGGGGHHHHH!!!!!
The very last boss fight against Paula isn't all that tough IF it's managed correctly. The problem is that ammo for the Dentist (machine gun) becomes somewhat scarce and while they provide ammo crates every minute or so, she needs to be killed within a certain time frame and you need a lot of ammo to do it. The key, I believe, is finishing her off in her 1st form with as much ammo left in your Dentist as possible. In the enclosed room where you first face her, there is an infinitely respawning blue ammo crate. Make sure you fill up the Dentist with as many rounds as possible before taking her out so that when you hit the check point, your gun has a lot of ammo left, preferably more than 150 bullets. That way you'll be able to widdle down her 4 wings quickly and the 50 bullets at a time you get from subsequent crates will be enough to defeat her. The first time I got to that checkpoint I only had 11 rounds in my Dentist and I always ran out of time and died because I couldn't damage her quickly enough. I reloaded the whole chapter, went into the checkpoint with plenty of bullets as described above and down she went, first try, bleep-bloop, achievement unlocked.
Hope this helps any who are struggling to complete this game on Hard. Seems a shame to have to endure all those cutscenes, likely for the 2nd time, only to not finish a game due to glitches that really SHOULD be patched.
Just like all bosses there will be 3 stages of his battle and will have to repeat it 3 times as well.
Damage, Stun/break apart, and finally focus on weak spot.
The first two times are not a problem, it is with the final attack is where a lot of people run into trouble.
Flemming will bring up a shield to protect him with 3 gaps intended for your SKULLFEST 9000's charge shot. Apon hitting him with this he will be stunned and ready for your Hot Boner. BUT due to a glitch the cracks will not appear. The game believes that you haven't done enough damage to trigger his weakness.
I have to restart the game to see if a maxed out Skullfest's damage is the key.
Most people that have this achievement had Flemming glitch when it was calculating damage.

White gems are crucial for buying red gems so I highly recommend farming them in certain areas. There are two locations where a drill demon thing comes out of the ground with three heads and you have to shoot the red glass in the darkness. The enemies here spawn infinitely as well does ammo so you can farm them. Enemies drop about 3 gems when killed so it will be a slow grind but you can also shoot out their legs and stomp them for extra gems. The third location you can farm white gems is chapter 3-2 when you're in the swamp. If you're following the red gem guide there is a log that needs to get destroyed to get that red gem and that's where you can farm white gems because 2 to 3 enemies infinitely spawn there and you can keep popping them. The max white gems you can hold is 999 so once you've got that proceed.
If you don't a lot of red gems it's not important but definitely helpful, but what you want to invest in is your weapons damage and capacity completely ignore reload speed. Obviously health and if you have extra gems I would say invest in the light shots size so it has a larger area of effect to damage enemies.
Most of the bosses aren't too difficult but quick mention of each before I talk about Fleming. The first boss, when he takes the goat head look in the sky a look for the red light that's where he's hiding. The reaper sisters shoot with the light shot, the second one is spotted quickest in the darkness by her lightly glowing red chest. The cartoon boss just keep shooting everywhere.
Finally Fleming, shoot him a lot and hit him with some sticky payloads. When he breaks apart use the dentist to lock onto him and spray. Same deal for the second round. The third round is tricky because he takes a lot of damage. So if you see he's about to put himself in the green glass stick him with a payload or two if you can. Once you shoot the skull inside charge up 4 shots again and shoot him once the glass breaks. This should deal a lot of damage and get you the yellow cracks. If not on the first try definitely the second.
For Paula lock on with the dentist, try to stock up on ammo while in the Fleming fight and before you leave the first room with her.

Story Related, Can't be missed
You are invincible during evade rolls... evade roll spam.
Spend more money on drinks, farm if possible, and focus on damage upgrades avoid charge speed upgrade it's pretty pointless.
Use your better judgement when trying to stomp enemies to farm or just kill targets to avoid damage, upgrades will prove very useful and you can abuse the checkpoint system but can be very costly. if you have to stun enemies crawling to you with your light shot and then allow one to get close for an easy stomp kill.
The Two spinning demons at the end of chapter 4-5 in the cave can be killed easily by evading out of the cave and rolling to the left causing them to crash into the mouth of the cave, flipping on their backs, allowing you to shoot them - finish them with the Skullcussioner.
In chapter 5-1+ to kill the Red tube electric guys just plant a bomb on them, with the hot boner charge shot, and shoot the bomb. it should take 2-3 charge shots. Evade > Charge > Fire.
I suggest playing on Lemon (Easy) first, then Demon (Medium) and lastly, Legion, just so you can get used to the slight increase in difficulty. If you still think you'll have a hard time, check my solution for 'High in Las Vegas' to earn unlimited Red Gems, which will allow you to easily and quickly upgrade your health and firepower.
- conserve ammo. a maxed out torch is a perfect way to kill without using ammo.
- never use alcohol- if you can. use the save points. if you're low on life, get to save point. save. quit. load up. life full.
- I've found that the upgrades to light-shot and reload time to be useless on the lower difficulties. But, on hard- those upgrades are worthwhile because of fleming fight. slow weapon reload time will slow you down. having a larger lightshot will help you with that goddamned swinging goathead.
- fleming: what a f---ing bast-rd. lots of guides about him. here are 3 difference-maker tips:
1) once he unconcsious- if is he does not have the yellow cracks, ignore him. he needs the yellow cracks at the START of his unconciousness, otherwise he will recharge. instead: use the skull-bazooka while he is standing. 2 or 3 skull shots. get in him to the yellow-cracks, then he will go unconscious. then drop a boner-bomb, and get into the flying-parts time.
2) the KEY here. KEY KEY KEY. lock on with the blue gun. don't try and chase the red part. get your distance. aim your blue gun. and follow the part, until it locks and turns red. then fire away. the bullets will hone in.
3) have a lot of alcohol. it may be a long battle. my hard-battle with him took 3 hours. yes 3 hours. until I figured out the body-part honing trick.
Difficulty in this game is split into the following:
- Lemon Hunter - Easy
- Demon Hunter - Medium
- Legion Hunter - Hard
Unlike other games, the difficulty related trophies in this game don't stack, so you'll need to complete this game a total of three times. There also isn't an New Game+ option unfortunately, so any upgrades you've done on previous will not carry over; you'll be starting from scratch for each playthrough.
Simply beat the game in the difficulties listed above to unlock their respective trophy. The trophies will unlock during the credits.
Tips:
- Aside from damage upgrades, don't upgrade your arsenal unless you really need to, particularly your health or a weapon's ammo capacity. Upgrading your health will automatically heal you to full, and ammo for weapons. Very useful if you want to save up on Drinks or White Gems
- Unless you're in desperate need for alcohol, save all your White Gems for Red Gems. You'll need all the extra money you can get especially if you're working towards High in Las Vegas
- Alcohol and Ammo can be found within the breakable barrels and boxes you come across in each chapter. For the sake of conserving ammo, break them open with your Hot Boner or the Light Shot
- Running low on ammo? Perform a Brutal Kill instead
- Weapons wise, focus on upgrading the damage before anything else. The Concusioner (and the upgrades after it) and the Torch alone will get you through most of the game
