How to unlock the Mercy Shot achievement in State of Decay - Definitive Guide
*SPOILER ALERT*. Around the time Lily's brother Jacob wants to relocate from the church the pastor will start to get sick, he should come down with black fever. When this happens move to the new house Jacob picks out for you. Now just do side missions going back to the house after each one and you should start to get updated on the pastor once going home. One of the guys in the house Alan will start suggesting killing the pastor if he doesn't get better and eventually he will. Just keep close by and keep doing side missions do not go to far or you can miss out!! Once Alan kills the pastor he will soon catch the same sickness black fever. Now go back to doing side missions in the area and you will soon get a mission called Mercy Shot. Now head home and talk to Alan, ask him to go with you on a walk to an isolated area and go take care of business and get your achievement :]
Now I just want to mention I've had the same thing occur in 2 play throughs but on my first play through I heard about the pastor getting sick but around that time is when you are driving far away to meet the Wilkersons or whatever their name is and you miss out on the updates because your not visiting the house and you will not get called about it. So to wrap it up, start your game and when the pastor get sicks do side missions in the area and keep checking in. Don't go far away and don't just stand around either, it's a waste of time. Not sure if my game just glitched, or if it's something to do with me having an infirmary in my base, but when I came back the Pastor was fine and no longer had black fever. So just to be safe I'd advise not building an infirmary.

I stayed at the church until Pastor Will died, and then I moved to the new house. Alan wasn't getting sick after doing a few side missions, so I backed out of the game to the menu, then to the dashboard, loaded up the game, walked back into the new house and Alan was sick immediately. On the way to a side mission the mercy shot mission popped, and BLAMO cheese mint :)

This achievement needs you to kill a member of your community who is injured or sick.
The game will some times make a character of your community hurt but waiting for this to randomly happen could take ages!
So what i did was select a character who is pretty useless (low skills) and then become them and run into a group of zombies to get your health down to the point where you fall on the ground and are told to tap A.
When this happens mash the A button to get back up and get the hell out of there and back to safety!
Once you get back, check your character it should say he is injured, then switch out to a different character.
After a while it will say if they are going to recover or not, if it says they arnt going to make it go back to the base.
When waiting to see if they are going to make it STAY CLOSE TO YOUR BASE! if your dont you will not get updated about them and they will be dealt with and you will miss out on the achievement.
Once it says the character is not going to recover head back to the base and talk to them. Tell them that we cant take the risk and you have to shoot them.
Take them to a isolated place and shoot them.

Its also a good idea to remove the medical area if you have one so they have less of a chance to recover

This is random rather than being a story achievement, as any member can get sick and start this mission. However, a story opportunity is likely to appear.
When a member of your community gets sick and fails to recover (medical area increases likelihood of recovery - but without it, they are probably more likely to just expire overnight without the opportunity for a Mercy Shot), a mission will appear at home base called 'Mercy Shot'. Nothing will be said on the radio, but briefly onscreen there'll be a note that X might not get better.
Go back home. When talking with the sick individual, you will get the choice of whether to execute or expel that member - choose to execute ("we can't take the risk."). If you miss an opportunity, don't fret - the same mission can appear for other members should they get sick.
I think you have to wait until the game prompts you with a mission to complete this, if anyone is *certain* that there was no mission at their home, but were able to Mercy Shot a sick member in dialogue anyway, please comment!

I have some evidence for the Pastor Will/Alan theory.
That said, I am pretty sure that I was never notified in my journal that anyone was ever sick (including Pastor Will and Alan).

This can also be obtained in the Breakdown DLC. Being you have to do this anyways in that DLC for I’d recommend working towards completing it there, rather than the base game. Either way, this achievement is a huge pain in the ass and will most likely waste a large amount of your time.
Note: It seems you’re only able to receive the Mercy Shot side mission for a community member who isn’t a Friend (the green bar to the right of their name in the journal isn’t full). Can’t confirm, but I had multiple friends gravelly ill and never received a Mercy Shot for them.
When you first start Breakdown, you’ll need to establish a home base by joining one of the optional communities. Once done, you’ll then be given side missions of sorts to rescue survivors. Keep rescuing survivors until you have some that are non-friends.
In order to get this achievement you must have a member of your community become sick (which you are able to view on the first tab of your journal; up on the D-pad). Once the member has become sick, you must then wait for the status under their name to change from whatever attitude (e.g. “Attitude: Assertive”) to “Fever.”
After waiting some more (shutting off your Xbox and going to sleep), if they don’t recover, their status will then change from “Fever” to “Gravelly Ill.” At this point, as long as it doesn’t state them as Friend, you’ll then be able to receive a Mercy Shot mission for them. If they’re “Away from Home,” try checking in on them with Y via the journal. Your map will then show their location, simply drive to them and select to send them home. After they’ve returned home you should receive the Mercy Shot. If not, just repeat the process.
To decrease the likelihood of recovery and increase the likelihood of the sickness spreading to other community members (which you want), you’ll want to avoid building a Medical Area. Also, empty your locker of all medical supplies. You want to do everything in your power to prevent recovery.
A quick way of getting a community member sick is to find a Bloater/Rotter (freak zombie that explodes into toxic gas which damages and can cause sickness) and driving into it with that member in the car. Then just let that member stand in the gas and it should cause them to become sick (get out of the car and they’ll do the same). Whether you choose this method or not, people should eventually get sick, esp. without a Medical Area/medical supplies.
When waiting for people to become sick, sickness to spread, or fevers changing to gravelly ill, the best thing to do is leave the game (i.e., turn off your Xbox, play a different game, etc.). Time progresses when you’re away from the game which is convenient for this. Waiting over night and returning the next day is a great way to progress the sicknesses as well.
Hope this is helpful in aiding towards this achievement. Again, you must do this in the Breakdown DLC if you want The Rescuer achievement. So if you’re unable to get it during the base game, don’t fret, just do it during Breakdown. Good luck and happy hunting!

At some point in the game (shortly after the Courthouse incident for me) one of your community members will be bitten and you'll get the Mercy Shot mission. Pick the top choice when you talk to them, and they will then direct you to the location on the map where they would like you to end them.
