I did this in movie night, and focused on saving all characters for the “everyone lives” achievement. I also picked options that maximized relationship status between the characters. I also picked up the gun earlier in the game. That being said, when I got to the chapter “low point” where the achievement is supposed to unlock when you instruct Andrew to wait for a clear shot, the achievement didn’t pop for me. The solution is much simpler than just picking head decisions. Once the rational trait appears on John’s trait screen, the trait/achievement will unlock after you perform another rational action. The key to getting this achievement does indeed lie within the chapter low point, and I will tell you a simple way of how to do it.
When you begin the chapter, John will say that he needs to have a break. Shortly after, your first dialogue choice as John will be either a head or a heart choice. The key here is to pick (HEAD OPTION) *Practical - none of us is safe.* This will update your bearings for the authority figure section and update it as “John was practical with the others about the danger they were in”. At this point in time, If you check John’s character trait screen you should see Rational in the background floating in and out of visibility. Once you see that this trait has appeared, John will have to select two more dialogue options before the encounter with Angela. It’s important here that you select “SAY NOTHING” both times because if you select any heart or head options here the rational trait will disappear from John’s trait screen. From here, you will instruct Andrew to “wait for a clear shot” when the figure appears from the mist, the achievement will immediately pop as you’ve selected two rational options in a row.

You should be able to get this during your Head playthrough; otherwise, aim to get it during your Save Everyone run. As the trophy suggests, avoid choices that steer towards believing something supernatural is happening, dismiss the paranoia and fear of the other characters, and you should get this by the time John and Andrew reach the museum, or while they're discussing the poppet, if you decide to interact with it.
