How to unlock the Embodiment of the Five Virtues achievement in The Witcher 3: Wild Hunt - Definitive Guide
The quest can be triggered by reading the “Test Yourself with the Trials of the Virtues” note on any of the notice boards within the city of Beauclair.
Once the quest is active, head over to the isle on Lac Celavy and speak with the Hermit. You will learn that in order to gain access to Aerondight you must first display the five chivalric virtues during your stay in Toussaint: Compassion, Generosity, Honor, Valor, and Wisdom.
Depending on when you choose to tackle this quest, you may have already completed some of the tasks and/or quests that would have already displayed such virtue. There are about three instances for each virtue, all of which are usually masked by timed dialogue choices you must make during quests. Make sure to read the inscriptions on the stones near the Hermit in order to get some clues for the virtues you might be missing.

Trial of Compassion
Main Quest: The Beast of Toussaint - After fighting the Shalemaar in the arena, you must decide to spare its life.
Trial of Generosity
Miscellaneous Task: Beauclair - After completing "Blood Run," enter the Main Square of Beauclair and a child will deliver a letter to you. Give him 5 crowns for his trouble.
Trial of Honor
Side Quest: The Warble of a Smitten Knight - After the horse race, choose to accept the mounted duel against the Rivian Prince and defeat him.
Trial of Valor
Side Quest: The Warble of a Smitten Knight - After following the Oriel (yellow bird) into the woods and returning to camp, you must decide to finish the tourney and go on to win.
Trial of Wisdom
Main Quest: La Cage Au Fou - Thoroughly explore the Wight's Lair and when hiding in the closet, try to lift the curse instead of attacking the creature.
As mentioned, there are a few more instances to display each virtue throughout the expansion, but the ones listed above are those in which you will come across the earliest. If you happen to miss any, you can choose to carry on and nab the other opportunities, or start the expansion from scratch from the main menu with a pre-made Geralt. It should only take about half an hour to run through the aforementioned quests and obtain all the virtues.
Once all the trials have been completed, return to the Hermit on Lac Celavy. To reclaim the coveted sword, you must first defeat the Hermit. Afterwards, the Lady of the Lake will appear and award you Aerondight, thus unlocking the achievement.
At the end of this quest, buy the painting from the artist for 1,000 coin.
To complete the Quest (Embodiment of the Five Virtues Achievement) you need to display the five chivalric virtues during your stay in Toussaint: Compassion, Generosity, Honor, Valor, and Wisdom. If you already completed a few Quests it's possible that you already have completed a few virtues, but it’s possible that you can’t complete the Quest since you chose the wrong options. Compassion:

- Contract: Big Game Hunter - After helping Beledar, agree to visit him at his feast. Wait until noon the next day, then head on over to the picnic. Ask about his daughter once the opportunity presents itself.
Generosity:
– Go to Main Square of Beauclair and a child will deliver a letter to you. Give him 5 crowns for his trouble.
Honor:
– The Warble of a Smitten KnightSide Quest: The Warble of a Smitten Knight – After the horse race choose to accept the mounted duel against the prince and win it.
– Side Quest: Goodness, Gracious! Great Balls of Granite!
Valor:
– Side Quest: The Warble of a Smitten Knight – After following the girl into the woods and returning to camp, decide to finish the tourney and win.
– Side Quest: Mutual of Beauclair’s Wild Kingdom – Turn down the extra gold at the end.
Wisdom:

If you have completed the five virtues go back to the Lake to complete the Quest, after that you receive the Aerondight Sword and the Achievement

You will see a ! near you when you exit, go talk to the boy and give him a tip for delivering you a letter.

When you talk to the bootblack, he'll ask for money to help his buisness. Give him the 500 coin.
-Mutual of Beauclair's Wild Kingdom
Let the Basilisk live (gives you Compassion) and refuse the additional coin from the nobles.
Valor
-The Warble of a Smitten Knight
You will have an option to stop participating in the tourney. Make sure you continue on and finish.
-Feet as Cold as Ice
After the Spriggen is slain, let the Knight keep the trophy.
-Fists of Fury: Toussaint
Beat all four opponents

Sparing the Shaelmaar in the Tourney.
-Capture the Castle (Main Quest)
Choose to help Roderick when he asks.

Free the panther from the trap and join Beledar at the feast and ask him how his daughter is.

You will be challenged to a duel before the third trial. Accept and win the mounted duel.

You must return Reginald's jewels to the Art Curator.
-Till Death Do You Part
Be honest with Charles at the end of the quest, and let him know it was only a marital spout.
Wisdom
-The Beast of Toussaint (Main Quest)
Late into the quest you will have to find three objects with the duchess. It will give you clues to the location where Milton is hiding. Guess greenhouse to get it right.
-La Cage au Fou (Main Quest)
Lift the curse of the spotted weight in the main quest. Make sure you choose the option to not eat with the spoon.
-Father Knows Worst
At the end of the quest, pick the choice to have the brothers make peace.
-Gwent: To Everything - Turn, Turn, Tournament!
Win the tournament
No big issue, but someone googling that might have issues
MISSABLE
This is part of the quest "There Can Be Only One". You will have to prove yourself in the 5 different virtues: Generosity, Valor, Compassion, Honor and Wisdom. You will prove this by doing quests throughout this game. It doesn't matter when you start this quest, as it will count any of the corresponding quests that you may have already completed. It is possible depending on your choice in certain quests, that you may not fulfil the 5 different virtues, causing you to miss out on the trophy, though you have multiple chances to fulfil the requirements needed for each one.
I had already completed 4 of the five once I started this quest and only had Wisdom left. Keep these virtues in mind when doing quests and think about the choices and how these virtues may fit in. Once you have completed the quests, you will have to prove your worthy in combat on top of the water, in which you will have to fight the hermit. Beat him and the sword is yours...again.

Generosity:
-Give 5 crowns to the boy who will deliver the letter from Yennefer/Triss at the main market of Beauclair.
-during the main quest “The Lair of the Bestia”, give financial assistance to the shoe shine boy, you can negotiate up to a little more than 400 crowns.
-In the quest "Portrait Artist" buy your portrait for 1000 crowns.
Honor:
-When completing the quest "People's Hero", you must return the stolen genitals of the statue to the museum worker, and not leave them to the thief for a fee. You can also borrow them and pick them up the next day.
-When completing the quest “Eternal Peace”, inform the customer that the cause of the noise in the cemetery was a family quarrel.
-In the quest “Song of a Knight’s Heart”, accept the challenge of the Prince of Rivia or Tyless (depending on the name under which your hero entered the tournament) and win.
Valor:
-Win a knight's tournament in the Quest "Song of a Knight's Heart".
-Become Toussaint's champion in fist fighting.
-In the witcher’s order “If you don’t want me to die...” after the fight with the monster, tell the knight-partner: “Take this head and return to Beauclair.”
-in the quest "Balance in Nature" refuse the order (receive "sympathy" and 350 crowns) and refuse the bonus.
Sympathy:
-Spare Charley in the arena while completing the main quest “The Beast of Toussaint.”
-In the quest "Beauclair Safari" do not allow the customer to get hurt. Then, after the end of the safari, ask the customer’s security guards about his life, then accept his invitation, visit the exhibition of his works, and once there do not miss the opportunity to inquire about the state of affairs of his daughter.
-After disenchanting the wight in the main quest "Echoes", a follow-up quest "The Hunger Games" will appear in a few days, where you must allow the disenchanted Marlene to remain in Corvo Bianco as a cook.
-When completing the quest “Balance in Nature”, refuse to kill the last silver basilisk.
Wisdom
-In the main quest “Echo”, disenchant the wight. To do this, inspect his kitchen before hiding in the closet, then decide to disenchant the monster, and after sitting down at the table with him, offer to eat without spoons.
-When completing the quest “Brotherly Love”, get the distiller part from the hot spring, and later, when Hugo asks you to protect him from his own brothers, answer that no one will die today.
-Win the gwent tournament in Toussaint.
Not everything is here, there are a lot of tasks and somehow you will perform the necessary virtue. At the end you will have a fight with the hermit (exactly the same fighting style as the Ophir magician in Hearts of Stone, only instead of sand there is water).
