How to unlock the Back from the brink achievement in This War of Mine: The Little Ones - Definitive Guide
Set the war to short, violence to low, and no winter. Make a custom map and make sure to include the "quiet house" and "garage".
Keep scavenging and feeding the group until the "quiet house" or "garage" is unlocked. Make sure to have a weapon crafted and go into the "house". Kill the old couple in there and loot the place with Boris. If you have "garage" you go in and kill the guy in the building. Be careful, he does have a gun. Once you kill the guy with a gun, go in the back and kill the old man.
Come back and Boris should be in "broken" state. Make sure to keep him fed and talk to him as the psychologist until he recovers. This worked very quick for me. It took me about 6 in game days or around 30 min.

Easiest way to get someone to a broken state is to kill 2-3 people. Then have another member of the group talk to them everyday till their mood rises. To speed along the process you can help out neighbors if those events pop up. The achievement will pop on the start of the new day where that character no longer has the broken status. Happy Hunting!
You will need to get someone back from having a "Broken" mental state in order to unlock this achievement. Several things will negatively effect a character's mental state. Losing a member of the house, killing someone who was not bad (as opposed to the thugs that might have occupied the Church, the criminals holed up in the Hotel and such), refusing to help someone in need, stealing or being deprived of their vice. It will take a bit to get someone to the Broken level, though.
The easiest way to get a person to the "Broken" level is probably to have them go on a killing spree. For a killing spree, the best place is the Supermarket with the Scavengers option. First, you will need a knife and a gun of some sort, preferably a shotgun with a lot of ammo. If you don't find a gun naturally, you will always be able to find a gun in the Abandoned Cottage. A note at the top of the stairs will tell you that a gun is buried in the yard. Then repair it at a Level 2 metal workbench.
When you enter the Supermarket, you will see three scavengers making plans to scour the building. After they split up, wait a few moments to ensure that you won't be disturbed, then stab the man on the main floor in the back and hope that you don't take too much damage before you finish him off. Then run up the stairs at the entrance, and see where the people are coming from first. The lady on the top floor may jump down through the hole in the roof. If she does that, stand to the right of the hole and shoot her as she lands. If they come up the stairs, stand to the left of the stairs and shoot them as they come up. You can also try to hide in one of the spots listed around the building and ambush them, if you prefer. If you survive this, you will come back broken, but you will not suffer any ill effects until you return home.
To fix someone who is broken, you will need to have another character in the house come over and talk to them. This will occupy both characters for several hours and hopefully the next day they will have progressed to a point where you can control them. Keep doing the same thing and talk to them again, and keep doing so until they are no longer depressed. You can also help improve their mental state by donating medicine at the hospital, helping your neighbors, or even sharing a drink of moonshine. Once the character is no longer Broken or Depressed, the achievement will unlock, but it will take several days of work.
This must be done on an adult character. Children have their own achievement for this called "No more tears."
