How to unlock the Was that supposed to hurt? achievement in Thronebreaker: The Witcher Tales - Definitive Guide
Read through the guide first before trying it and it does require you to understand how the game plays as I cannot explain that to you in a guide quickly.
Here's how I finally did it:
First of I started a new game on the easiest difficulty so you can skip battles to get to the one battle I wanted.
I never changed any card in the deck or upgraded them in the camp. I used the starting default deck.
To skip a battle you need to forfeit the battle while in it then the game will ask you to press A to retry, Y to reload last checkpoint, X to skip battle and B to go back to main menu.
Obviously you press X to skip the battle.
At the start of the game skip all battles until you get roughly half way around the map until a mission called TRAPPED turns up. When there's dialog with the main character and her enemies always choose to fight. As your decisions effect the outcome of the story. You need to do this as the mission you want for this achievement is a story mission so you need to do what I did to make sure the mission turns up.

If you have looked at notice boards on the way it's a point of interest on the map north of the dragons head which is on the bottom of the map.
When the fight starts you need to make sure you have certain cards in your starting hand. These cards are the 2 War wagons and at least 1 Lyrian arbalest. So replace the cards until you have those 3 there.
If you don't get them all in your hand at the start of the fight forfeit the match and restart to try again.
Once they are all in your hand you are ready to attempt the achievement.
The enemy always goes first by playing a card face down followed by a unit card. The card that's face down is your only problem in getting this achievement. The card is called a Nilfgaardian interceptor which is the only card the enemy has that can damage you. So we need to get rid of it as soon as possible.
To do this you need to play your cards in a very specific order at the start of the match.
So when its your turn the first card placed should be a war wagon.
Then on your second turn should be another war wagon.
Then on your third turn you play the Lyrian arbalest to destroy the Nilfgaardian interceptor. You need to manually aim the card so you don't hurt yourself.
All the above cards should be played on the opposite row to which the Nilfgaardian interceptor spawns on. So if the Nilfgaardian interceptor is on the melee row you place your cards on the ranged row. If the Nilfgaardian interceptor is on the ranged row you place all your cards on the melee row.
Doing this stops the Nilfgaardian interceptor damaging you when its timer expires. which in turn lets you attack it an destroy it instantly.
With all the above cards on the same row your Lyrian arbalest has 7 attack power to kill the Nilfgaardian interceptor instantly.
While playing your cards don't worry about any other card the enemy plays as they are not important at that time.
After the Nilfgaardian interceptor has been destroyed no other unit can damage you so all you need to do is have a higher attack value at the end of the round to get the achievement.
You will need to figure this bit out for yourself as the order of cards is played randomly from here on out and your cards in your deck will also be random, So as I said earlier you need knowledge of playing the game to do this.
On the plus side this fight is not hard and very basic for the Gwent games.
As a little tip and warning take your time with any attacking card as it's so easy to attack yourself and void the achievement.
The achievement unlocks on the victory screen so wait until it unlocks. If it does not press the guide button and quit the game and try again.
This is the way I unlocked this achievement so I hope it helps someone.
This might not work for everyone as the achievement is buggy but I thought I would write something new for you to try.
MISSABLE
In Lyria, after you pass Rastburg, you’ll run into a fast travel point. Take the path to the right and there is a point of interest where you’ll get ambushed. There is only one card the ambushers have that can damage you which is the Nilfgaardian Interceptor.
It is a shortened battle, and at the start of it, you’ll want to make sure you have three specific cards: 2 War Wagons and a Lyrian Arbalest. The other cards don’t matter. When you start the match, the card facing down is the Interceptor. Whichever row the card is on, you’ll want to play your cards on the opposite. Your first two turns, you’ll want to play both wagon cards and on the third turn the Interceptor should face up, where you can play your Arbalest card to destroy it. After this, make sure your point value stays higher than your enemy and win. No other cards can harm you.
This achievement is likely to be unlocked through natural play, especially when playing on easy. This is best attempted against an enemy that does not deal considerable damage and instead attempts to earn a high score to win. You can unlock this by using the same battle that can be used for the "I have the power!" achievement, so feel free to refer to its description above for a location.
What you need to do differently for this one, however, is to change your cards to be offensive or damage dealing cards as opposed to high score cards. Have your leader weapon as 'Meve: Rivian Broadsword,' which will have your leader ability damage all enemies equal to the power of a chosen enemy by 4. This is very useful to have. Then, have 'Scepter of Storms' as your trophy; this will allow you to damage enemies by 2 each turn, or you can save it up and use them all at once after a few turns to deal 2 damage multiple times. Next, you will want 'Zoria Runstone,' 'Cadaverine,' and 'Alzur's Thunder' among your trinkets and/or any other offensive trinkets that you have or prefer.
In your deck you will also want damage dealing units such as 'Strays Bomber,' 'Rivian Sapper,' 'Lyrian Arbalest,' 'Reinforced Ballista,' and 'Rivian Onager,' among others. Then what you will need to do is try to damage the enemy by using your damage dealing cards while playing as little of your units as possible. So play all of your trinkets and use your leader ability to keep the damage up and only play your units when there is little enemy presence on screen to limit their ability to damage you.

One of the conditions is not to change the composition of the starting deck after the training is completed, because... I fulfilled the conditions for the trophy more than once throughout the game, but the trophy never came out.
You can get it after passing in the first chapter, we go further than the middle and after the castle we turn onto the eastern path, the squad is attacked from an ambush. There must be two squads with carts and at least one crossbowman in your hand; if the cards are not knocked out, we go into the game and give up, then we replay until we have the necessary cards in our hand. We look in which row the AI puts the closed interceptor card, put our cards in the opposite row (we put the interceptor in the melee row in ranged combat and vice versa, so that the interceptor cannot attack), it is very advisable to place the crossbowman in the filled row at the first opportunity and in one kill the invader once. After killing the invader, not a single card can cause damage, and we replay someone else’s deck based on points.
