Done in TR3 - Area 51.
The first four guards you encounter in the stage all have access to Handprint Scanners that Lara is unable to use, and they each will stop you from getting items/a secret.
I strongly recommend NOT trying to go for this achievement on your Pistol only run, particularly because the 4th guard is just far too close to the hand scanner to easily kill him before he reaches it (I'm not entirely sure if it's even possible?).
Guard 1 is right at the beginning. When you start the level, draw your weapons and run across the room. The Guard will bolt over to the left and the end of the corridor. If you kill him in time, you'll be able to access the room at the end that contains the MP5 Submachine Gun and 2 sets of clips for it.
Shortly after this is Guard 2. You'll head through a crawlspace with the red laser thing moving across a vent. As you're coming out of this crawlspace, you will see the guard walking towards the right. If/when he spots you he'll run in that direction, and on the other end of that hallways is the handprint scanner. If you kill him in time, he won't trigger the green lasers that span across the hall.
Guard 3 is shortly after this hallway. As you go through the room with the cieling turrent and green lasers (these are permanent and not triggered by the previous handprint scanner) you'll turn left and head up a ramp. At the top of this ramp is a guard who will bolt to the other side of the corridor. If you kill him in time, you'll be able to access the room on the right that contains the Harpoon gun, Grenades, and a Rocket.
Finally, Guard 4 is not far from this area, though I'd save your game now if you haven't already as this one is easy to miss. Continue through the corridor and pull the lever on the wall. This will cause you to drop down and into the outside area. Equip a weapon stronger than your pistols and carefully head to the right next to the black slidey pillar you came from. Look round to see where the guard is. As he gets close to where you are, come out and shoot him quick before he makes it to the handprint scanner.
If you kill him in time, you will not have two dogs attacking you, and you'll be able to reach the first secret below the grating on the right, containing the Save/Collectible Crystal.
If you've killed all of them in time, the achievement should unlock immediately after killing the 4th Guard.

This must be done in Area 51. Kill the first 4 guards you encounter before they reach the handprint scanner. Use the Desert Eagle and be quick: they don’t actually have to touch the scanner to activate it, if they get close enough it will count as touched too, especially #3, which is the tricky one. If you don’t find the room with the harpoon open, it means the guy “touched” the scanner, therefore the achievement won’t unlock. I hope this helps someone!
