How to unlock the Fully Prepared achievement in Warhammer 40,000: Rogue Trader - Definitive Guide

After you wake up in a pile of bodies after the "trial" you'll start with 4 blue debuffs and one Fresh Injury. Look around the area to find perception checks. If you pass the check you'll find various items, the ones you're looking for are Stimms which will heal one trauma each. Not sure if it's a bug or not but I only needed 3 Stimms, leaving me with the Fresh Injury and one blue debuff called A Hundred Wounds. The Hundred Wounds doesn't seem like it can be removed as I had a fourth Stimm but it didn't have an effect. I also found a Battle Stimulator Medikit for the Fresh Injury but I couldn't use it so it seems like it intentionally doesn't count toward the achievement.

Sorry, I was working from memory when I typed up the solution. If I remember correctly, you'll start on a triangular platform and for the purposes of direction let's say the bottom of the triangle is south and the camera is pointed north. There should be a pile of trash at the bottom of the stairs on either side of the triangle.
