How to unlock the Twisted and about to break achievement in Darkest Dungeon - Definitive Guide

To get this you need to complete a dungeon after all four of your heroes over 100 Stress, and each one needs to fail the sanity check to get an affliction. Some of the possible afflictions are paranoid, selfish, masochistic, rapturous, etc.

If one of the heroes hits 100 Stress and gains a virtue instead it won't count.

Be warned, when a hero accumulates 200 Stress, they have a heart attack and die. So they won't count for the achievement.

If you don't get this through normal play and want to force it, I did this with an Abomination hero in the party, and went into a short dungeon.
The Abomination has an ability that allows him to transform into a beast and while the Abomination is a beast he will inflict Stress upon all the heroes in the party.
This allows you to apply Stress on everyone rather evenly, and quickly, and you should be finished with a short dungeon before any of them have a heart attack.

The Abomination information is somewhat incorrect. Abomination only deals stress damage to the party during the initial transformation, every turn from then on the Abomination will take 5 stress damage but the party won't take any more.

Abomination can even end up being a massive hindrance. With their decent crit rating they can end up healing more stress for themselves and the party than they cause.

Sorry for the delayed response...

You have a good point.
When I used the abomination I mostly skipped attacking, and either used his turn to move him in the lineup, or have him transform into human/beast. I had the rest of the party do the actual killing. This way he wasn't getting crits that healed stress, and tended to not give himself too much more stress than the party.

Also, some classes refuse to be near the abomination, limiting the types characters that you can bring with you.

I havnt tried this but was thinking if you complete a dungeon and remain afterwards, you could then enter a corridor go to the end and then retreat back and forwards gaining stress until you all hit 100

Confirming that Stoney's suggestion above works, I just unlocked by getting my 4th affliction via walking backwards after the quest was completed.

Get All Four Heroes Stress Level To Over 100 To Trigger Afflictions.

*Please Note*
If Someone Happens To Become Virtuous Instead, The Achievement Is Voided.
- Just Make Sure Not To Equip Any Plus To Virtue Chance Gear And You'll Get It Eventually.

Achievement Pops When You Leave The Dungeon.

During your quest your heroes will get stressed. Stress damage is black and sums up to a total stress level of the hero. A “clean” hero has 0 stress (stress level 0), during his quest he’ll get stress damage which varies in value and source. Once the stress hits 100 he/she needs to do a sanity check in which he can gain an affliction, with a chance of becoming virtuous instead. If a hero has an affliction status his stress will increase more rapidly and possibly cause stress to others.

Complete any quest with 4 heroes who have 100+ stress and have lost the sanity check (and so suffer from an affliction).

You clear the dungeon and you can simply walk around it in circles, it is advisable to move backwards - it is more likely that the characters will be “afraid”. As soon as everyone has received the fear effect (the white strip under the character is filled with hollow squares - 1 level of psychosis), you can press “start” and go to the city. The trophy is yours.

Regarding the advice above, if you hold down + , you can put out the torch ( + light). In the absence of light, the fear scale fills up faster, but there is a greater chance of running into a fog. Don’t forget about the collector either; he appears randomly with a small chance if the inventory is more than 65% full.
Received a trophy after one of the boss battles on the 5th level of the dungeon. Following the advice above, I went for walks several times, but instead of “psychosis” I received “emotional elation” from one of the heroes.
