The first 4 story chapters are the only chapters available without advancing the campaign. The achievements required are listed below. Along with the chapter it is unlocked in.

You may start work on this achievement before that time though as the individual achievements are in the journal with the exception of "Nostalgic."
You must earn all relevant "in game" achievements to earn this achievement! (credit: Littlejay)
You are able to replay chapters at any time (once you've completed them at least once) by visiting Lord Neverember in Protector's Enclave.
Chapter 1: River District Unveiling: Water Weight: Upon entering the River District for the first time, if you turn around you should see a sparkling box; this allows you to pick up some water balloons (you do not have to equip them). Directly behind that, towards the water, are some targeting dummies. Throw a water balloon at them by pressing , and you'll receive the in-game achievement and a new title.
Chapter 2: A Crown for the King: Hope Over Experience or Experience Over Hope: You only need 1 of the in-game achievements. At the conclusion of the chapter you will speak to Nostura; after the conversation ends (choice of dialogue does not matter), a chest will appear as she leaves. If you open the chest you'll receive 'Hope over Experience' and a new title; if you leave through the gate on the left you'll receive 'Experience Over Hope' and a new title. Either way you choose you're going to get an in-game achievement.
Chapter 3: Going Undercover: Kind of a Big Deal: When you approach the door to infiltrate the hideout, the guard will tell you to get lost. Select either yellow text option to get the next speech option. Do not select an orange speech option! Select the white speech option that reads: "Out of curiousity, you sound like you don't believe in (player name)". The DoorMan goes on and you should choose: "I wouldn't doubt (player name's) power, Ive heard all the legends are true". You'll then see two more speech options and you should choose the orange/blue text option that reads: "I'll show him a thing or two later; Yes, I'll come in". Upon selecting that option the 'Kind of a big deal' in-game achievement will unlock.
A Good Person, A Great Person, or Just Alright: You must complete the previous step to earn this next in-game achievement; you only need to earn 1 of the 3. Progress through the chapter until you have to fight through the recruits and escape. When you reach the last group of recruits, the door guard will be the last enemy you fight. When you defeat him, you'll be able to speak to him, do so and you'll have a couple of options. (Leave the bouncer be) will grant you 'A Great Person'; (Kick the bouncer) once will give you 2 more options of (Leave the bouncer be) again, which will grant you 'A Good Person' or (Kick the Bouncer one more time) and then (Leave the bouncer be) you'll get 'Just Alright'.
Chapter 4: The Missing Link: None
River District: Upon completion of chapter 4 you'll be able to explore this new zone. There are many things to do and see, including delving into the new 'vaults', upgrading guard towers and new Heroic Encounters.
Currencies: You'll now need to start earning those campaign currencies. You can get them multiple ways and there are daily missions given by Ostaram Copperwise.
Mercantile Missions: daily quest, you can earn 25 EoE and 4 SoMS
There is a secondary daily mission, there are 6 to choose from; they are: The Departed, Hidden Cache, Magic Recovery, Heirloom Weapons, Family Heirlooms, and Secure the Area. Most of these require you to plunder a vault. Make sure you loot the alternate chest!
You'll eventually be able to complete a weekly quest given by Kavatos Stormeye that will net you 20 more EoE.
Abandoned Treasures: Earn these from vault clears or Heroic Encounters in RD.
Evidence of Evil: Earn these from daily and weekly quests and Heroic Encounters.
Secrets of the Many-Starred: Earn these from daily and weekly quests; but they are only used for boons.
You must complete the following Campaign tasks to further the story!
Heir Apparent: 36 Abandoned Treasures, 30 Evidence of Evil
Investigate Kabal: 48 Abandoned Treasures, 120 Evidence of Evil
Confront Kabal: 56 Abandoned Treasures, 120 Evidence of Evil
Part of the Party: Unofficially added to the party of Adventurers: When you first enter the zone and jump down a few platforms you'll come across a group of people on a platform. One guy has a fire protection spell and he decides to test it. Needless to say it doesn't work out so well. After his misfortune and the party gathers near the edge; listen to their conversation and you'll be "added" to their party. This will unlock the in-game achievement.
Too Hot to Handle: Survived Kabal's toughest challenge in his trial: Once you pass the last of the flamethrowers and reach a campfire, you have the option to cross the bridge to confront Kabal. Instead, go left from the campfire and discover another set of platforms you can jump across. Make it to the end and earn the in-game achievement.
Investigate Nostura: 48 Abandoned Treasures, 120 Evidence of Evil
Confront Nostura: 56 Abandoned Treasures, 120 Evidence of Evil
Fey Marked: Avoided the dangerous runes racing to Nostura's Fey Castle: You must avoid all the glowing runes that are along the track on the ground. Utilize the Stag's speed boost by pressing to jump them if you have too. You might be better served attempting this solo.
Highly Skilled: Avoided Getting hurt by Nostura's dark magic spheres racing to her Fey Castle: You must avoid the pink spheres that come flying towards you, or travel across the track. You might be better served attempting this solo.
Investigate Gyrion: 48 Abandoned Treasures, 120 Evidence of Evil
Confront Gyrion: 56 Abandoned Treasures, 120 Evidence of Evil
Better by Leaps and Bounds: Completed the most challenging leaps in Gyrion's plane: After the second set of jumping puzzles, and you are about to confront Gyrion for the final time, go back out of the room with the camp fire. On the outcropping, look to your right and down and you should see another platform below you. This will be tricky, because you'll have to jump and then use the leap ability to reach the platforms. But should you die, you'll be right at the last campfire. Reach the final platform and earn the in-game achievement.
These campaign tasks will take at least 30 days to complete. You might be able to shave off a day or two with the weekly quest as well as farming Heroic Encounters for Evidence of Evil.
Chapter 5: Ascendancy's End: Nostalgic: Upon the defeat of Gyrion, inspect the sparkling barrel nearby; this will trigger a flashback of sorts and netting you the in-game achievement and a new title.
You can track your progress by visiting your Journal, tab to achievements, scroll down to the "Quests" sub-section and look for "Far Realm Denier". It will list all the in-game tasks you've finished and what you're missing. When you finish everything, you'll receive the Far Realm Denier and Ascendant Triumph in-game achievements as well as the Xbox achievement.
1, At the very beginning of the campaign, the first chapter, upon arrival in the quarter, you need to turn around, take a balloon with water and throw it at the scarecrow.
