Can confirm that on my playthrough doing all side quests before finishing the campaign (and picking up all journals on the floor that I noticed), I was at 296/300. I believe that each expedition gives a journal plus some of the new boss enemies in them do too. No need to specifically grind for this one it seems!

Quick advice: If you have collected most journals and are not getting the unlock, while in the journal screen hold y to mark all as read. This may fix the glitch that does not credit some found journals as counting in your found total.
For me, I was stuck at credit for 270 journals though I had completed the game and was only missing 10 total entries. I started a new character and in that account marked all journals as read so I could keep track of any new ones found that I missed. After marking all as read the notification popped for multiple journals found and the achievement popped right after. Hopefully this will fix anyone else with the same glitch. Please post in the comments if this trick works for you. It is possible the glitch will get fixed in a future update.

There are a total of 339 journal pages in the game. You will automatically unlock at least 250 journal pages during the story and all side-quests, so you'll need to pick up at least 50 of these when going through the missions. Ones that must be collected will have the same blue glow as ore deposits, and will also have pages floating up into the air like embers on a fire. These are not very hidden; so long as you explore the edges of all the mission areas, you'll find enough.
There are a grand total of 339 journal pages to collect in the game but you luckily will only need 300 for this trophy, giving you some room to miss a few here and there. The majority of these will be picked up automatically through general story progression and completing the side quests for Squaretasking . 92 can be picked up in the open world and the rest will be from the Expeditions. Below is a list of all the pages you'll pick up in the open world. They'll for the most part be pretty straightforward to find based on the map but if they're a little bit more hidden, an image will be provided so you can narrow it down. They will appear in-game as pieces of paper that are floating and glowing blue.
Rift Town (11):

- 1: Hounds Note - On a box down the 1st set of stairs, just behind two guys looking down over the railing.
- 2: Flores Flight Reports - Just next to a canister-like object inside the main "shop" area.
- 3: The Landing: Just to the left of the door you exit to leave the Camp area.
- 4: Textile Manufacturing - To the left of the door you exit to leave the Camp area, on some shelves on the far side of the room.
- 5: The relocation - Just to the left of the Weapons dealer in the shop area.
- 6: Shira's Journal: Downstairs from Shira's Office (go past her office).
- 7: Eva's Letter: In the "A Bad Day" Quest Area, it's inside a container just to the left of a door you need to unlock. There is an ammo box just to the right of it
- 8: Barker's Journal - As soon as you finish the "A Bad Day" side quest, you'll be in Mr. Lopez's room. Before you leave, the page will be on a table.
- 9: Feeding the Colony - When you leave the camp, immediately to the left will be some stairs. At the top of the stairs is the page.
- 10: The Coming End - Just after the first fast travel point, there will be an ammo box among some rubble from a building. Just behind the box is the page.
- 11: Reiner's Journal - On a crate in the room where you fight Reiner.
The First City (18 - while the number ticker shows 18, there are only 17 locations. One of the bastards counts as 2 but there are multiple where you pick up 2 so as long as you pick up all "17", you'll get them all):
- 1: Powering the Cryopods - When you first start in this area, you'll make your way up a grassy hill before entering a small town-like area. Before going inside, the page is going to be on a body in the grass, just the left of the "town" entrance.
- 2: Solar Panels - After you cross the bridge and make your way up the large anomaly hill, you'll be an open area with some cement barricades where a handful of enemies will spawn. The page is just next to one of these barriers before you go through the next gate. It's by one of the sparky cylinders.
- 3: Formation of the Outriders - After the fast travel point you reach where the road splits up towards a Historian Side Quest item and left for story progression, take the left path and as you're going down the hill, the page will be on the right just next to a body.
- 4: The Outriders Revamped: When you reach the area with the snipers and the gallows, just before you climb up and over the scaffolding the snipers are perched on, the page will be inside a building with some fenced up windows.
- 5: Landmark: Radio Tower - Shortly after the previous page, you'll have to make your way through a small building and back outside, then up a hill to the left. The page is at the top of the hill, next to a fence, between two metal structures.
- 6: Disbanded - When you reach the large tree with the bodies, the page will be on the ground just in front of it.
- 7: Setting up the Colony - After the previous page, you'll come to an area where you have to fight through a few waves of Perforos (this is a decent location to go for Misfortune Loves Company if you haven't earned it yet). Before breaking through the white rubble, go to the far left side of this area and just next to a red and white trailer, up on a metal walkway, will be the page.
- 8: Anomaly Storm & The First Anomaly Storm - Just to the left of the breakable white barrier (same one as above), inside a container.
- 9: Jane Colm's Journal - In the area where you have to lower the bridge after defeating a few waves of Perforos, go up the last hill on the right (just before the bridge) and at the top, underneath a scaffolding, will be the page in the grass.
- 10: The Storm's Aftermath: On the ground, just to the left of the box you have to interact with to lower the bridge.
- 11: Exiles - Before you cross the bridge, climb up to the top of the tower just on the right side of the bridge to find the page.
- 12: First Citizens - Keep progressing forward and you'll eventually get a sub-quest to activate the crane. From the valve you have to turn, go back down one set of stairs and at the far end of this raised walkway will be the page.
- 13: Anna McKellan's Journal - This is picked up during the "No Place Like Home" side quest. When you reach the old man's home (and clear it), this page will be in the home with the word "THIEF" written on the side of it.
- 14: Cryopods - Picked up during the "Endless Dark" side quest. It'll be inside the medical lab.
- 15: Transfer Order & Nathan Scurlock - Picked up during the "Endless Dark" side quest. It'll be inside the medical lab.
- 16: Mass Awakening - Picked up during the "Endless Dark" side quest. In the last outside/open area before you enter the lab with the Cryopods, the page will be on the ground just to the left of the entrance to the lab.
- 17: Report on the Fungus & ECA Research Report - Picked up during the "Endless Dark" side quest. It'll be inside the darkroom with the Cryopods at the end of the quest.
Eagle Peak (10)
- 1: Flores Engine Report - In your basecamp, on the opposite side of the way you move forward to progress the story.
- 2: The Second Anomaly Storm - When you take the lift down to progress the start of the story-related quests, you'll come to an area where a few enemies will spawn. When they're down, you'll need to interact with a gate to open it up. Before doing so, climb up to the top of the structure just in front of the gate and there will be a small camp area with the page.
- 3: A Foreign Agent - After progressing the story a little bit, you'll get a sub-quest to unlock a door. When you do, there will be a fast travel point just on the other side. To the right of the travel point, tucked away in a small hidden area, is a camp with a crate and the page next to it.
- 4: Locked In - Progress the story forward some more and you'll make your way through a handful of enemies before you reach a building on top of a hill. Go inside and the page will be sitting on a table.
- 5: Jocelyn Dunham's Journal - After you take the next lift down to a new area, you'll make your way through some enemies and eventually come to a building with a large solar panel in front of it. Before going inside. there is another building to the right (near a row of a few more solar panels) with some stairs that you can take to get inside. The page will be on the ground in the corner.
- 6: Tracing the Signal - Go inside the building the solar panel in front of it (to progress the story) and before you go out through the door on the other side, the page will be tucked in a corner to the left.
- 7-10: Alchemist's Log x4 - These will all be part of "The Scientific Method" side quest and will all be shown as quest markers on your map so you can't complete that quest without picking these up.
Deadrock Pass (4):
- 1: Alastair Cullen's Journal - Inside your camp, on the opposite side to progress the story forward, tucked in the very back corner.
- 2: The Crisis - While still in your camp, before crossing the first bridge, go inside a container on the right and the page will be on the ground.
- 3: The Great Extinction - Just to the right of the previous container are some stairs. Go up them and then turn around to the left. The page will be near the end of the short walkway.
- 4: A Joint Effort - Picked up during the side quest "A Free Market". When you enter the quest area, you'll come to a fast travel point. Just behind this will be a short hidden pathway leading to the page. If you're facing the way forward to progress with the quest, it'll be on the left side.
Trench Town (5):
- 1: Hanged Man's Letter & World in Anarchy - Just next to the camp area, in a room with some shelves and tires (mostly tires). It'll be on the back left shelf.
- 2: Lucy's Private Letter - On the crate just behind Lucy. This is incredibly hard to miss as you'll need to talk to her to pick up a side quest required for Squaretasking .
- 3: The Formation of the Insurgents - Go past the entrance to the bar and follow the path as it goes left. It'll be on the left side, on a table in a small structure.
- 4: Children of Enoch - On a body just in front of the large doors.
- 5: Peace Negotiations - Cross the bridge going opposite of the bar and turn right towards a group of people watching a fight. Make a hard right when you get to them and the page will be at the end of the small walkway (it'll be a dead end).
Wreckage Zone (7):
- 1: Greybeard's Journal - As you make your way through Clearwater Springs (it'll be the left path in the first tunnel you enter into), you eventually come to a fast travel point and the bones you need to interact with to pick up the Hunter quest in this area. Continue forward and the path will split (left will take you to the Hunter area and right will progress the story). Go right and just on the left side by some rocks will be the page.
- 2: Bruno Nyberg - When you fast travel back to camp, go back through the starting tunnel and this time turn right (towards a dead end). The page will be on the ground at the end of the tunnel.
- 3: The First Altered - As you make your way through the Polluted Outpost, go left of the gate leading to the next area and follow the path all the way down. The page will be on the ground on the left. There will also be a crate and an ore node in this same area, just as a reference.
- 4: Power Struggle - Go through the gate to progress the story forward from the previous page and you'll come to a fast travel point. Just behind the flag, on the ground next to a body, is the page.
- 5: Building the New World - Progress forward with the story and you'll eventually enter a building where you need to follow a wire on the ground. In the second actual room you enter (not the small hallway), the page will be on the ground in the back left.
- 6: Seth - This entry is story-related. You'll automatically pick it up when you clear the path while in the Irradiated Shores.

The Quarry (10):
- 1: State of Emergency - When you first start this section, you'll need to clear the swamp so your crew can make camp. Keep going until you reach your first fast travel point. There will be a door you need to enter that takes you to "The Outpost". Ignore this and head towards the large door (if it's closed, it'll be a dead-end, if you've already completed this part, it'll be open). On the right side just before the door will be the page.
- 2: Victor Corrigan's Journal - When your crew has made camp, it'll be on the table just to the left of Channa.
- 3: The Floating City - Just next to the camp, take the path just behind Zahedi (it'll be opposite of story progression) and there will be a wooden ramp leading up on the left side. About halfway up this ramp will be the page.
- 4: Boris Morrison's Journal (#1) - This will be in the side quest area for "The Outrider's Legacy" Clear the path at the top of the wooden ramp from #3 (leads to 'Nesting Grounds'). Make your way through the enemies (if you haven't already cleared them) and just before the gate you go through to actually reach the hatch, on the ground on the right will be the page. It's just next to a flaming barrel.
- 5-8: Sergio Acosta's Journal, Sarah Tanner's Journal, Boris Morrison's Journal (#2), The Nuke - These are all found inside the Outrider's Hatch. Each door in the hatch will open up as you pick up keys throughout the game. The 1st door you can find Sergio's, the 2nd door has Sarah's and the last two are picked up when you open the 3rd door.

- 1: Enoch Biodiversity Report - Inside the camp. Run just past where Jakub/Channa is sitting (to his right) and the page will be on the ground on the left side.
- 2: The Takeoff - On the ground just next to Mercer Acosta, inside the Enclave Camp (you need to speak with him to get a side quest).
- 3: Mercer Acosta’s Journal - Just next to the previous page, behind Mercer, on the outside of his little camp area.
- 4: Juno's Journal - From the previous two pages, progress forward towards the gate that leads to the Enclave Outpost. Before you reach it, there will be a set of stairs leading up to a slightly higher area on the left. The page will be just next to an enclave member.
- 5: Sergio Acosta's Note - This will be in the side quest area for "The Outrider's Legacy". With the quest active, you'll get a map marker to find his (Sergio Acosta's) body. The page is in a small brown box just by the body.
The Stronghold (8):
- 1: Zahedi Report - Inside the camp. One a table just to the left of where Channa/Jakub is standing.
- 2: Forest Enclave Established - Go towards the main story marker ("Save Zahedi") and you almost immediately come to an area with a large truck sitting right in front of you. To the left of the truck, on a red blanket of sorts, will be the page on the ground.
- 3: Ticket to the Flores - After you rescue August, you'll need to clear out another camp of enemies before you can progress forward. When they're all gone, you'll come to a fast travel point. Keep going forward and you'll have to take out a named enemy. When you go, an area-objective (blue gear/circle) will appear telling you to open the gate. Before you do, to the right of the gate are some stairs and to the right of those is a building. Go inside and the page will be on a table on the right.
- 4: Kang's Journal - You'll need to pick up the wanted poster (it's just next to the valve you turn to open the gate mentioned in #3) so the "Maneater" area opens up. When you have access to it, head towards it (Drenched Caverns) and continue forward until you get the area objective to lower the bridge (it'll be after you have to clear the caverns). - 1: Sarah Tanner's Journal - As you progress through "The Outrider's Legacy" questline, you'll eventually find yourself in The Gate looking for Sarah Tanner's body. Her body will be marked on the map as a quest objective. Just to the left of her, in front of a tent, will be the page.
- 2: Enclave's Smuggler Report - In the same area as the previous page. On the opposite side of the area, just to the right of a crate (it'll be a little bit up some stairs).

If you miss it on your first run through that quest, you'll need to pick it up again before the area will become available.
